Job Description :Under the general direction and supervision of the Life Enrichment Manager, coordinates the Special care Unit workflow and activities for all shifts. The Dementia Program Coordinator works collaboratively with the Interdisciplinary Team to oversee and supervise all aspects of care/programming for the dementia resident. Participates in care plans and staff meetings. Organize and plan, with the assistance of Social Services, meetings to address the needs/concerns of residents and their families. Provide education and training to all staff as regards to dementia, activity based programming and behavior management. Ensures appropriate documentation is initiated and maintained throughout the resident’s stay.
