Clinical Care Navigator
JOB SUMMARY:  Under the general direction and supervision of the Chief Nursing Officer the Clinical Care Navigator is responsible for clinical management of patients across the continuum. The Clinical Care Navigator will guide patients, families and caregivers to informed decision regarding care transitions/Acts as a patient advocate/case manager for hospital/clinic patients. Assist with patients returning to the community from other facilities with care transition. Applies review criteria to determine medical necessity for admission and continued stay. Provides clinically based case management, discharge planning and care coordination to facilitate the delivery of cost effective quality healthcare and assists in the identification of appropriate utilization of resources across the continuum of care. Function as a liaison for management, staff, providers, insurance companies, community organizations and other customers Participates in quality improvement and evaluation processes related to the management of patient care. Education/Experience:  Current Montana RN license. Three years acute care hospital experience. Management experience preferred

Rehabilitation  Director
Under the general direction and supervision of the CEO, is continuously responsible for managing the Physical Therapy Department, including planning, organizing, directing and supervising department functions, activities, and employees. The Rehab Director has primary responsibility for budget development and monitoring, provides input for compensation decisions, development of new programs or initiatives, policy and procedure development and/or revision, hiring, training, retention, performance evaluation, and disciplinary actions for employees and contractors working in the department. The Rehab Director ensures clinical, operation, and financial success of the Rehab Department, meeting the financial and clinical goals of responsible areas.  Responsible for integrating all appropriate standards into operations.  Collaborate with all responsible team members-Medical Director and CEO.  Education/Experience:  Certified/licensed/registered healthcare professional with a minimum of a Bachelor’s degree (Master’s preferred), five years of clinical experience, and at least two years of management experience Previous experience in administrative management preferred. Experience working with large organizations or systems preferred. Functional knowledge and experience with teamwork dynamics and the ability to work in a strong team atmosphere.

 

 

 Sleep Lab Technician
The Sleep Lab Technician performs standard Polysomnography procedures on patients in sleep lab, to include preparing patients for tests and observing patients during tests, making treatment decisions including the application and titration of oxygen and/or PAP therapy per department protocol. Works under the general supervision of the clinical director (M.D., D.O., or PhD) or designee to provide comprehensive evaluation and treatment of sleep disorders. This may involve polysomnography, diagnostic and therapeutic services or patient care and education.” Education/Experience:   High School Diploma or equivalent is required. General knowledge of the specialized field of Polysomnography, EEG Technology, Nursing, Emergency Medical Technologist (EMT), or Respiratory Care, which could be acquired from a two-year program preferred. Three to six months of “Allied Health” experience necessary.  Successful completion of an accredited educational program leading to an associate degree with an emphasis in polysomnography or Respiratory Therapy.

Under the direction of the Director of Nursing, the ECF Nurse Supervisor is responsible for the effective implementation of the philosophy, goals, policies, and procedures of the extended and the nursing department and their effect on patient care in the department. This is a working position with rotation of holidays and weekends. Education/Experience: Current RN licensure with previous supervisory experience. Five years of clinical experience preferred. Other qualifications/certificate/license:  State of Montana Nursing License

 

 

HOMEMAKER POSITION
Work when your needed position for Homemaker to perform routine household tasks in client homes on a scheduled basis.  Required duties include: light housekeeping, vacuuming, sweeping, mopping, dishwashing, meal preparation, laundry, and grocery shopping. Must have transportation to to/from client homes, be able to safely operate client-owned machines/equipment and be able to work independently in the home setting. Education/Experience:  Must have experience in home management and performance of household tasks, including the purchasing of food and preparation of meals.  Other qualifications/certificate/license:  Must have current MT state driver’s license and a personal vehicle in good working order.

 

 

Fitness Specialist-Full-Time
Performs Wellness Center functions to facilitate member’s fitness. Participates in process of assisting members and provide exercise specialty classes.  Education/Experience:  High degree of knowledge in fitness, strength training, flexibility, cardio, & balance activities, along with interpersonal skills, anatomy, & medical terminology. The ability to obtain a Wellness Coach Certificate within first six months of employment

 

 

Home Health Associate Part Time
Associate will do general office duties. Must be computer literate and work with deadlines as well as work with a variety of departments/clients to coordinate services, visit, charge sheets, progress notes and physician orders. Completes monthly and annual statistical report of home health agency visits and home maker reports for the North Central Area Agency on Aging. Encodes and transmits OASIS documentation per federal regulations. Education/Experience High School graduate or equivalent. Previous office experience in a health care delivery setting; familiar with accounting procedures is preferred.

 

 

Medical Billing Specialist
Conrad employer hiring full-time Medical Billing Specialist. Requirements: High School diploma; minimum of 1 year long term care medical billing experience, 1 year exposure to CPT codes, HCPC codes and Revenue Codes as they relate to a UB04 and HCFA 1500 claim forms, and 1 year exposure to healthcare collections; basic understanding of long-term care Medicare and Medicaid billing regulations; knowledge of laws, regulations and guidelines pertaining to long term care administrative procedures; ability to tactfully handle collection in a resident focused environment; highly developed clerical skills; good problem solving and analytical abilities; dependability; flexibility; excellent computer skills to include Word, Excel, Outlook, and internet; ability to maintain records, balance books, and general accounting procedures; strong organizational and communication skills; ability to take initiative, work well under pressure, handle multiple tasks, and is detail-oriented. Staff Paramedic-Full Time
Responsible for responding to requests for ALS interventions and/or patient transfer for providing emergency medical care in the pre-hospital environment according to paramedic protocols and policies approved by the hospital and the ALS system medical director.  Performs other duties related to the provision of care and development of the community EMS system. Education/Experience:  Completion of paramedic level training required; Minimum of three years’ full time experience preferred as a provider of pre-hospital care in an emergency setting; .Paramedic experience in a two-tiered non-transport ALS system is preferred; full time experience must be within one year of date of application.
Other qualifications/certificate/license:  Current EMT P registration in the state; Current certification in CPR, American Heart Association Course C or American Red Cross CPR for the Professional Rescuer; Current certification in Advanced Cardiac Life Support, Pediatric Advanced Life Support, American Heart Association; Current motor vehicle operator’s state license

 

 


PHYSICAL THERAPIST POSITION FOR HOME HEALTH-IN GLACIER COUNTY
Part-time Home Health Physical Therapist (P.T.) to assume responsibility for the provision of skilled Physical Therapy services under a physician’s Plan of Care in the homes of Glacier County residents. Must have good assessment and teaching skills, the ability to implement in-home therapy treatments with accurate, timely documentation, and regularly evaluate the Plan of Care to achieve positive patient outcomes.  The P.T. will be accountable to the Home Health Manager but works cooperatively with all providers and disciplines and is responsible for the supervision of Physical Therapy Assistants in directing the P.T. treatment program.  Must be currently licensed as a Physical Therapist in the State of Montana.   Technical Skills:  Time Management, Customer Service, PT evaluation, Plan of Care development and implementation, Verbal and Written communication skills, Physical Therapy modalities, Therapeutic exercise and Manual Therapy Skills.  Job Requirements: Bachelor’s Degree in Physical Therapy (PT), 2 years of clinical experience with varied patient population preferred, Current MT PT License and CPR Certification.

Job Summary: The  ward clerk/CNA through communication, requisitioning and management of supplies and equipment, assists the activities of the nursing staff, the patient, the physician, other hospital departments and visitors. He/she performs general receptionist and clerical duties in order to facilitate efficient functioning of the nursing unit. Participates in assigned patient care duties. JOB REQUIREMENTS: Must have current Montana C.N.A. License or obtain within 6 months of hire date and BLS required

 

 

RN/LPN Restorative Position

Under the general direction and supervision of the Director of Nursing, Extended Care, coordinates and performs quality projects, analyzes department needs, supervises restorative staff,coordinates restorative program through interdisciplinary team approach. Acts as liaison/advocate for residents. Supervises and participates in the activities of the Extended Care Department to assure the deliverance of quality care, promotion of comfort, maintenance of functional abilities and respect for resident’s dignity. Will be a supplemental instructor to the CNA program, offers educational in services and performs skills teaching to nursing staff and CNAs. Education/Experience:  Current Nurse license for the State of Montana. Attends continuing education programs in order to maintain licensure .Demonstrated proficiency in nursing, knowledge and skills
